You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may, however, may require you to complete a self-certification form SC1 which is available from your employer.
Some employers insist on a Doctors Sickness Certificate for the first seven days, if this is the case then a private sick note can be issued for a fee of £10.00.
For any illness lasting longer than six days or more you will need to see the doctor for him or her to assess the medical condition and to issue a sickness certificate if necessary.
For on going sick notes you do not need a GP appointment. You can contact the surgery on the date the sick note will run out and inform our reception team who will send a message through to the GP.
For return to work notes PLEASE NOTE THESE CAN ONLY BE ISSUED ON THE DATE OF THE RETURN TO WORK.
